3 What information does Hamers Edge Pty Ltd collect about you?
3.1. Clients and prospective clients
(a) When you enquire about our services or become a client of Hamers Edge Pty Ltd, a record is made which includes your personal information.
(b) The type of personal information that we collect and hold will vary depending on the circumstances and the services requested, but may include:
(i) your name, date of birth, contact details and other identifying information;
(ii) information about your employer or an organisation you represent, and your professional details;
(iii) NDIS-related information, including your NDIS participant number, plan details, funding arrangements, plan management arrangements, goals and nominated representatives;
(iv) information about your disability, physical or psychological health, functional capacity, support needs, treatment, medication, assessments and services provided or proposed;
(v) information about your language, communication requirements, cultural background, family, carers, guardians, nominees and other members of your support network;
(vi) service agreements, appointment information, case notes, reports, correspondence, incident and complaint information, and billing and payment information;
(vii) images, audio or audio-visual recordings where relevant and collected with appropriate consent or other lawful authority; and
(viii) any additional personal information you provide to us, or authorise us to collect, as part of your interaction with Hamers Edge Pty Ltd.
(c) Some of this information is sensitive information under the Privacy Act, including health information and information about disability, racial or ethnic origin, religious beliefs, sexual orientation or criminal history. We collect sensitive information only where it is reasonably necessary for our functions or activities and with consent, unless collection without consent is required or authorised by law.

If you are a customer or participant in an event, we may disclose your personal information to our clients and venues where reasonably necessary for and relevant to delivering the event.
We will only use an identifiable image, audio recording or audio-visual recording for promotional purposes with appropriate consent or where otherwise permitted by law. Where the individual does not have capacity to provide consent, consent will be obtained from an authorised representative where legally appropriate.
9.3. Disclosure to contractors and other service providers
(a) Hamers Edge Pty Ltd may disclose information to third parties engaged to assist us in providing services or operating our business. With appropriate consent, Hamers Edge Pty Ltd may use an AI-assisted clinical documentation tool to process audio from a consultation and generate a draft transcript, case note or other clinical document.
The tool may process personal information and sensitive health information discussed during the consultation. We use that information for clinical documentation and the provision of services and not for unrelated purposes.
AI-generated material is treated as a draft. An appropriately qualified worker reviews and, where necessary, corrects the material before it is relied upon or added to a client record.
We do not use AI-assisted clinical documentation to make automated decisions that could reasonably be expected to significantly affect an individual’s rights or interests.
10. How can you access or seek correction of your personal information?
(a) You are entitled to request access to personal information held about you by Hamers Edge Pty Ltd. To request access or correction, contact our privacy officer using the details below. We may need to verify your identity before providing access or making a correction.
(b) We will not charge you for making an access or correction request. We may charge reasonable, non-excessive costs of giving access and, where practicable, will advise you of any charge before providing access. We do not charge for correcting personal information.
